Procedure - Evisceration
Performed by - Dr. Bowden
Surgery time - 26 minutes
Following induction of anesthesia, the feathers surrounding the
right eye were removed. The right eye and surrounding area were
prepped using diluted betadine solution. Once in surgery, the
dorsal and ventral palpebrae were removed using iris scissors.
A blade was used to remove the cornea. A surgical spatula was
used to remove the lens and vitreous. Sterile cotton tipped applicators
and sterile 4x4 gauze was used to remove the remainder of the
tissue from the interior of the orbit. The third eyelid was left
undisturbed within the orbit. Following removal of the tissue,
excessess palpebral tissue was removed, and 5-0 PDS suture was
used to close the palpebrae in an intradermal pattern.

OD: Non-functional, 2 + blephorospasm, phythisical eye postition, soft globe, vitreus present/4 + flare/4+cell/admisced floculent fibrin /hyphema/hypopyon in AC, green hazy vitritis PC, diffuse corneal edema, 1+ dense corneal edema ventrally, 4+ mydriasis with posterior synechia and rubeosis iridis. Fibrin and pigment on ALC. Cannot view fundus.

Tx Plan:
Recommend future evisceration OD, give OS time to determine long-term function. 
Ketorolac TID 
Continue Optixcare BID OD and Meloxicam BID 
Recheck in 7-10 days.
